Resumen: El control clínico y la monitorización de la bilirrubina en la etapa neonatal son fundamentales para evitar la toxicidad sobre el sistema nervioso central. Objetivo: determinar la correlación entre las cifras de bilirrubina transcutánea (BTC) y bilirrubina sérica total (BST) en recién nacidos ≥ 35 semanas. Pacientes y Método: estudio observacional, transversal, analítico y retrospectivo que incluyó a 90 neonatos de edad gestacional ≥ 35 semanas e ictericia mucocutánea a los que se les realizó BTC y simultáneamente BST, entre el 1 de junio de 2022 y el 31 de enero de 2023. Se compararon ambas variables y se determinó su correlación. Resultados: se analizaron los indicadores de validez obteniendo una sensibilidad y valor predictivo negativo de 100%. La media de las determinaciones de BTC fueron de 14,84 mg/dl ± 2,27 y la de BST de 13,1 mg/dl ± 2,39. La correlación obtenida indica que ambas variables están relacionadas siendo ésta una correlación directa y en función de la ecuación de predicción se manifiesta una apropiada correlación entre ambas. Se determinó que la BTC sobreestima en el 95,56% de las determinaciones la BST, e infraestima en el resto (4,44%). Las mediciones simultáneas de BTC y BST fueron distintas en todas las determinaciones con una media de las diferencias de 1,72 ± 1,48. Conclusiones: el método no invasivo BTC puede utilizarse como herramienta de cribado inicial para la población neonatal ≥ 35 semanas, dada la adecuada sensibilidad y valor predictivo negativo.

Abstract: Clinical control and monitoring of bilirubin in the neonatal stage are essential to avoid toxicity in the central nervous system. Objective: to determine the correlation between transcutaneous bilirubin (TcB) and total serum bilirubin (TSB) levels in newborns ≥ 35 weeks. Patients and Method: observational, cross-sectional, analytical, retrospective study that included 90 neonates of gestational age ≥ 35 weeks with mucocutaneous jaundice who underwent TcB and TSB measurement simultaneously between June 1, 2022, and January 31, 2023. Both variables were compared, determining their correlation. Results: the validity indicators were analyzed, obtaining 100% sensitivity and negative predictive value. The mean of TcB determinations was 14.84 mg/dl ± 2.27 and that of TSB was 13.1 mg/dl ± 2.39. The correlation obtained indicates that both variables are related, which is a direct correlation and, according to the prediction equation, there is an appropriate correlation between them. It was determined that TcB overestimated TSB in 95.56% of the determinations, and underestimated TSB in the rest (4.44%). Simultaneous measurements of TcB and TSB were different in all determinations with a mean difference of 1.72 ± 1.48. Conclusions: the non-invasive TcB method can be used as an initial screening tool for the neonatal population ≥ 35 weeks, given its adequate sensitivity and negative predictive value.

Abstract: Introduction: gestational diabetes is one of the most frequent endocrinological diseases in pregnant women, which has implications for newborns. Objectives: to describe the type and frequency of complications of newborns born to mothers with gestational diabetes, as well as to determine if metabolic control during pregnancy modifies the frequency of these complications. Material and methods: observational, comparative, and retrospective study, carried out in two years (2019-2021). Data from mothers and newborns were recorded. The comparison of the variables was performed with the Student’s t test and χ2. Results: during the study period, there were 2,827 births, of which 216 were newborns born to mothers with gestational diabetes (7.6%). Of these 216, 22.7% required admission to the neonatal unit, with respiratory distress syndrome, jaundice, and hypoglycemia being as the most frequent causes. Nine newborns with obstetric trauma (4.2%) were registered, and an equal number with some type of malformation. When comparing neonates with adequate metabolic control during pregnancy (n = 186) with those with inadequate control (n = 30), it was found that in the latter group there was a higher frequency of respiratory distress, jaundice, hypoglycemia, obstetric trauma, and congenital malformations (p < 0.05). Conclusions: the frequency of complications of newborns born to mothers with gestational diabetes is higher when there is inadequate metabolic control during pregnancy.

Resumen: Introducción: la diabetes gestacional es una de las enfermedades endocrinológicas más frecuentes en embarazadas y tiene implicaciones para los recién nacidos (RN). Objetivos: describir el tipo y la frecuencia de las complicaciones de hijos de madre con diabetes gestacional, así como determinar si el control metabólico durante el embarazo modifica la frecuencia de las complicaciones. Material y métodos: estudio observacional, comparativo y retrospectivo, realizado en dos años (2019-2021). Se registraron datos de madres y de RN. La comparación de las variables se realizó con prueba t de Student y χ2. Resultados: en el periodo de estudio hubo 2,827 nacimientos, de los cuales 216 fueron hijos de madre con diabetes gestacional (7.6%). De estos 216, 22.7% requirió ingreso a unidad neonatal, donde el síndrome de dificultad respiratoria, ictericia e hipoglucemia fueron las causas más frecuentes. Se registraron nueve RN con traumatismo obstétrico (4.2%) y un número igual con algún tipo de malformación. Al comparar a los RN con control metabólico adecuado durante el embarazo (n = 186) con los de control inadecuado (n = 30), se encontró que en este último grupo hubo mayor frecuencia de dificultad respiratoria, ictericia, hipoglucemia, traumatismo obstétrico y malformaciones (p < 0.05). Conclusiones: la frecuencia de complicaciones en RN hijos de madres con diabetes gestacional es mayor cuando no hay un adecuado control metabólico durante el embarazo.

Abstract: During the SARS-CoV-2 pandemic, efforts have focused on trying to identify the routes of transmission of the virus, characterize its symptoms and signs, and investigate the best diagnostic and therapeutic methods. There are fewer published data and series in the pediatric population than in adults. Objective: To analyze the clinical and epidemiological characteristics in children under 16 years of age diagnosed with SARS-CoV-2. Patients and Method: Descriptive study carried out on children who underwent SARS-CoV-2 RNA testing due to compatible symptoms, close contact, or requiring hospitalization or surgery, in the Emergency Department of a hospital in Madrid, Spain. 30 variables were collected including epidemiological data, symptoms, and signs of infection. Results: Out of 1378 patients, 12% were positive (165). There was a higher proportion of patients of North African origin in the positive group than in the negative one (p < 0.01). Of all patients, 35.6% reported close contact with a confirmed case, which was more frequent in the positive group. 75.8% of the positive patients had some symptoms, most frequently fever, runny nose, and cough, followed by digestive symptoms. There was one case of COVID-19 pneumonia and two patients with MIS-C, one of which had SARS-CoV-2 infection. Eight of the positive patients (4.8%) required hospitalization due to SARS-CoV-2 infection. Conclusion: Although SARS-CoV-2 infection is milder in the pediatric population, almost 5% will require hospitalization. No close contact was identified in a high percentage of patients (61%). Further studies are needed at all levels of care to characterize the infection in children and adolescents.

Resumen: Introducción: Durante la pandemia por SARS-CoV-2 los esfuerzos se han centrado en intentar identificar las vías de transmisión del virus, caracterizar clínicamente la infección e inquirir los mejores métodos diag nósticos y terapéuticos. Los datos y series publicados en población pediátrica son menores que en adultos. Objetivo: Analizar las características clínicas y epidemiológicas de pacientes diagnosticados de infección por SARS-CoV-2, menores de 16 años. Pacientes y Método: Estudio descriptivo en un Servicio de Urgencias de un hospital de Madrid, España, de los menores a los que se realizó test de RNA SARS-CoV-2 por síntomas compatibles, contacto estrecho o por precisar hospitalización o intervención quirúrgica. Se recogieron un total de 30 variables que incluían datos epidemiológicos, síntomas y signos de infección. Resultados: De 1.378 pacientes, el 12% (n 165) fueron positivos. En el grupo de positivos hubo una mayor proporción de pacientes de origen norafricano que en el de negativos (p < 0,01). El 35,6% de todos los pacientes refería contacto estrecho con un caso confir mado, siendo más frecuente en el grupo de positivos. El 75.8% de los pacientes positivos tenía algún síntoma, entre los más frecuentes se registró fiebre, rinorrea y tos, seguidos de síntomas digestivos. Hubo un caso de neumonía por SARS-CoV-2 y dos pacientes con SIM-P, demostrándose en uno de ellos infección por este virus. 8 pacientes de los positivos (4.8%) precisó ingreso debido a la infección por SARS-CoV-2. Conclusión: Aunque la infección por SARS-CoV-2 cursa de manera más leve en la población pediátrica, casi un 5% precisará ingreso. En un alto porcentaje de pacientes (61%) no se identifica un contacto estrecho. Son necesarios estudios a todos los niveles asistenciales para caracte rizar la infección en niños y adolescentes.
